About The Duke Center for Onco-Primary Care provides evidence-based, patient-centered, personalized health care across the cancer continuum – from cancer screening to survivorship. The clinicians partner with oncologists and primary care teams to provide whole-person care. Job Location The APPs associated with the Duke Center for Onco-Primary Care Program will practice at several locations as needed DCI Cancer Center Durham, Women’s Cancer Center Macon Pond, Raleigh, Duke Primary Care Sites, Duke Family Medicine Center and/or telehealth. After a period of on-site training, this role will be a hybrid position, with 1-2 days per week working remotely. Job Summary Survivorship Patients Provides direct patient care in outpatient settings for adult cancer survivors. Develops individualized survivorship care plans to address the long-term and late effects of cancer treatment, counsel and screen for treatment-related sequalae, monitor for cancer recurrence or secondary cancers, and address psychosocial concerns. Patient population includes adults age 18 and older, with a primary focus on breast cancer survivors and adult survivors of childhood cancer, as well as survivors of other cancer types based on clinic needs. Multi-Cancer Early Detection Partners with patients in shared decision-making regarding multi-cancer early detection testing.
