Assures efficient patient utilization of continuum resources through close communication and collaboration with the patient, primary treatment team, insurers, and community and family support systems. In collaboration with the treatment team, plans appropriate individual and group treatment plans to meet the ongoing needs of assigned patients.Job Description:Primary Responsibilities:Provides Assessment Completes a psychosocial assessment within department guidelines for timeliness and content. Aims to understand and identify presenting problems and aftercare needs through input from the patient, family, outpatient providers and collaterals as the patient allows. Identifies the patient’s strengths and challenges as they pertain to managing symptoms and adherence to an aftercare plan. Identifies when there is a need for a family meeting and/or a network meeting. Provides Casework, Intervention and Aftercare PlanningAttends and participates in treatment team meetings, contributing to the clinical understanding of a patient’s presentation. Shares recommendations for aftercare, progress towards arranging aftercare and identifies obstacles to discharge.
